Yeovil Town F.C. will be running open trials for our Under 12 age group.
The trials will take place on the 11th & 18th March between 5-7 PM.

Player Name
Player DOB
Contact phone number and email of the applying player’s parent /guardian
Players current Club and contact details of a club representative
